Reporting to the Manager of Rehabilitation and District Stroke Services, the responsibility of the Registered Practical Nurse (RPN) is the assessment and care of, and the treatment of health conditions by therapeutic and supportive means to Rehabilitative patients in order to attain or maintain optimal function. Care is provided to support patients and families within an inter-professional context with a large focus on patient specific goals and family participation. Rehabilitation is dedicated to improving, maintaining or restoring physical strength, cognition and mobility with maximized results. The RPN will work within the full defined professional scope of practice as set out by the CNO.

QUALIFICATIONS

Required:

  • Ontario College of Nurses registration or eligibility
  • Demonstrated Ability to work at the full scope of practice as defined by the CNO and required by department
  • Recent Experience (within the past three years) working on in-patient Rehabilitation Unit
  • Successful completion and demonstrated skill in Supported Conversation Training
  • Successful completion of NDT (neurodevelopmental treatment) course
  • Recent experience working in a leadership role
  • Dysphagia Screening
  • Functional Independence Measure (FIM) certification
  • Demonstrated awareness, understanding and ability to facilitate principles of rehabilitative care
  • Demonstrated excellence in patient/family education
  • Excellent problem-solving, decision-making and interpersonal skills
  • Medication certification and ability to practice
  • Physical Assessment Course or equivalent
  • Intravenous certification
  • Proficiency in power chart and power orders
  • Excellent organizational and patient assessment skills
  • Excellent communication and collaboration skills with patients, families and other healthcare professionals
  • Commitment to continuing education and professional development
  • Experience in unit specific Quality Improvement project
  • Recent satisfactory performance and attendance records
