STATUS: PART-TIME
WAGE RATE: $30.49 – $35.15
Reporting to the District Stroke Manager of Grey and Bruce, the Rehabilitation Facilitator is responsible for implementing goal focused interventions in the home and various community settings under the direction of and in collaboration with Allied Team members. Specific areas of program focus include cognitive and communication strategies training, independent living skills, physical functioning and facilitating community integration. The Rehabilitation Facilitator will also provide client-specific consultation and education/training/in-services related to stroke rehabilitation for service providers, caregivers and families. The Rehabilitation Facilitator is a member of a collaborative interprofessional team delivering an innovative rehabilitation service to help stroke survivors and their caregivers reach their highest potential of recovery in their own community.
Qualifications / Skills / Abilities:
Required
- University degree in a social science area and/or college diploma in a field such a OTA/PTA, Social
Service worker or Developmental Service Worker - Current Basic Cardiac Life Support certification
- 3 years recent experience and expertise in working with individuals with Stroke
- Knowledge of stroke rehabilitation and current stroke rehabilitation best practices guidelines
- Experience working in an inter-professional team
- Sound understanding of the role of a variety of rehabilitation professionals in the provision of stroke rehabilitation
- Experience and expertise working in community
- Working knowledge and experience with theories and principles of guiding adult learning and patient self-management approaches
- Excellent problem solving skills with demonstrated creativity, innovation especially when solutions are not obvious
- Excellent communication, interpersonal, organizational, problem-solving and innovation skills
- Willingness to work flex time to meet patient needs
- Proven flexibility and a high level of initiative
- Appreciation of resource utilization
- Ability to utilize Cerner, Microsoft Outlook, Excel, Word, and Adobe
- Valid Class G driver’s license and willingness to travel
- Ability to work independently
- General skills and abilities in the areas of facilitation, teaching, collaboration, written and verbal communication
- Recent satisfactory performance and attendance record
